Privacy and Confidentiality Policy.
This service will maintain confidentiality and privacy of all associated persons and related records as per the 13 Australian Privacy Principals which were outlined in the March 2018 reforms to the Privacy Act 1988.
This service will collect information from you and your referring practitioner (when applicable) in accordance with this agreement and our privacy and confidentiality policies. Your personal information will be stored securely and electronically using the Halaxy Practice Management Software on a password protected computer only known to your treating practitioner. Should any breach occur involving your personal information, you will be notified as soon as reasonably possible. If you would like to access your personal information, a request must be made in writing and will be responded to in a reasonable time frame. The information provided to you will be in a hard copy and there may be an administrative fee charged for this request. You may be refused access to your personal information if there is a serious threat to life, safety and health of any individual, or to public health or safety at any time.
